WP1. Scoping review of evidence for digital PHDRS in social care

We are conducting a scoping literature review of academic literature and literature from wider sources (e.g., project evaluation reports published by social care services) to identify the current state of evidence for post hospital discharge services using technologies and provided within social care. The review will (1) identify and describe the range of technologies and service models provided by social care services; (2) describe the evidence that currently exists for these services and (3) identify factors that influence the delivery, implementation and scaling up of these services across social care.
