WP4. Participatory Theory of Change (ToC) development

This work package will bring together findings from previous parts of the project to create a participatory theory of change. The theory of change will form the main form of results from the project and will set out the inputs, assumptions and contextual factors, mechanisms of change and outputs and outcomes associated with each participating service. These will then be brought together to create an overall theory of change for post hospital discharge services using technology in social care.
