WP2. Mapping and analysis of social care service data within post hospital discharge services

The technologies deployed in post hospital discharge services can collect large amounts of service level data about clients.  With new technologies being integrated into services, there will also be greater potential for service to collect further data which can be used to improve services. However, services don’t necessarily know what data they collect, or how this data could be used to identify outcomes for clients and then be used to improve service provision. This work package will explore the scope, range and types of data routinely collected by technologies provided within each service, to determine whether services can use this data as a means to monitor and then evaluate service provision, so they can determine if they are meeting their goals.
